You can download the form at the Taoyuan E-Services Portal (https://e-services.tycg.gov.tw/) under Labor Services > Employment Discrimination Complaint Cases. Upon receipt, the City Government will investigate based on the facts, interview both parties, and submit the case to the committee for deliberation.
