1. Long-term care services in Taoyuan City can be applied for through the following methods:

(1) Telephone Application:
You may call the 1966 Long-Term Care Service Hotline to apply for services.

Service hours:
Monday to Friday
08:30–12:00 and 13:30–17:30

Please prepare the following information in advance:

• Name of the person requiring care
• Age
• ID number
• Residential address and current condition
• Name and phone number of the primary caregiver/contact person

(2) Fax Application:
Please complete the “Taoyuan City Elderly Frailty Assessment and Long-Term Care Service Referral Form” and fax it to 03-3321338.

(3) Online Application:

(4) In-person Application:
Applications may also be submitted at district offices, health centers, hospital discharge planning service units, family service centers, the “Comprehensive Service Counter” on the 1st floor of the Taoyuan City Government Building, the Department of Social Welfare, and the Department of Public Health.
