Applicants must meet one of the following qualifications:

(1) Dementia patients aged 50 or above
(2) Indigenous persons aged 55 or above with disabilities
(3) Disabled persons with physical or mental disabilities
(4) Dementia patients
(5) Individuals who, during the assessment period, meet the eligibility criteria for the Post-Acute Care Integrated Care Program announced by the National Health Insurance Administration, Ministry of Health and Welfare
